Occupational Therapist (OT) – Home Health
Location: Middle and Northern Butler County

Job Summary:
As an Occupational Therapist (OT), you will provide skilled occupational therapy to patients on an intermittent basis at their place of residence. Care is provided according to physician orders and individualized plans of care, under the supervision of the Director of Clinical Services/Clinical Manager.
Responsibilities:
Conduct initial evaluations and develop individualized plans of care
Provide skilled occupational therapy and perform all prescribed procedures
Reassess patient progress every 30 days and update care plans as needed
Consult with physicians regarding treatment changes and progress
Instruct patients and families in home exercises and functional programs
Supervise Certified Occupational Therapy Assistants and Home Health Aides as assigned
Participate in patient care planning with other home health staff
Document visits, progress notes, and discharge summaries accurately and timely
Present in-service training to agency staff and participate in QAPI activities
Qualifications:
Graduate of an accredited Occupational Therapy program and certified by the National Board for Certification in Occupational Therapy (NBCOT) or eligible for National Registration Examination
Licensed in Ohio (or relevant practicing state)
Minimum of 2 years’ experience preferred
Strong communication skills and ability to work independently
Commitment to improving patient outcomes and quality of care
Working Environment:
Travel to patient homes within Butler County
Work indoors in office and patient homes
Physical activity including standing for extended periods and moderate lifting may be required
Supervision:
Reports to: Director of Clinical Services/Clinical Manager
Supervises: Certified Occupational Therapy Assistants and Home Health Aides as assigned
