WCS Healthcare is currently seeking an Case Management Nurse for a contract assignment at a Prestigious Hospital in Northampton, MA.

Job Function:

\n

• Day Shift.
\n
• 8am to 4:30pm.
\n
• Assess new patients.
\n
• Update and revise patient health care plans as needed.
\n
• Educate patients and families about health care plans and options.
\n
• Make clinical decisions for routine patient care.
\n
• Accurately document and submit medical records.
\n
• Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.

Job Requirements:

\n

• Graduate from an accredited School of Nursing or Bachelor's Degree in Nursing (BSN).
\n
• Current RN license for the state that you are working in (or willingness to obtain).
\n
• BLS certification from the American Heart Association.
\n
• CCM certification preferred.
\n
• 2 years of acute care experience in this specialty.
\n
