Full Time Permanent Licensed Practical Nurse

 

Job Summary: The Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) provides nursing care to patients in their homes as prescribed by the physician, in compliance with applicable laws, regulations, and company policies.

Reports To: Case Manager/Clinical Supervisor

Responsibilities:

• Drive to patients' residences.
• Ensure quality and safe delivery of home health services, including patient, family, and caregiver education.
• Implement the plan of care initiated by the Case Manager.
• Provide accurate and timely documentation consistent with the plan of care.
• Monitor patients' conditions, including fluid intake and output, and maintain patient charts.
• Assist patients with daily activities such as eating and bathing.
• Administer injections, prescribed medications, enemas, etc.
• Clean and dress wounds and assist with other basic medical care tasks.
• Collect samples for lab testing as assigned.
• Participate in the coordination of home health services, reporting the need for additional disciplines (CNA, OT, PT, MSW) to the Case Manager/Clinical Supervisor.
• Use equipment and supplies effectively and efficiently.
• Attend case conferences and training as required by the agency.
• Perform other duties as assigned by the RN.
• Perform duties consistent with the Nursing Practice Act, including the Standards of Competent Performance.

The salary is negotiable and will be based on experience.
